All classic players (teams U11 & older) AND challenge players (teams U13 & older) need to do the following paperwork for NCYSA registration:

All TU classic and challenge players - ONLY please do the following NCYSA registration paperwork
(1) NCYSA Medical Release Form - need 2 copies - 1 copy must contain original signature The office has copies of the NCYSA form in duplicate, or click on this link to fill out and create 2 copies 2011-2012 NCYSA Medical Form  (print, fill out, & make 1 more copy, as you need 2)
Team names must follow the following formats:	Classic team name examples: "93 TUSA Silver" - boys teams "93 TUSA Silver G" - girls teams	Challenge team name examples: "U14 TUSA Liverpool" - boys teams "U14 TUSA Fire G" - girls teams

All Triangle United players (YDA, Challenge, Classic) need to fill out the following forms and provide the following information to the club each year.

All Triangle United players please do the following registration paperwork

(2) copy of birth certificate - only needed for players NEW to Triangle United

(3) Parent/Player Pledge - must be signed by parent and player, and returned to the TU office Parent/Player Pledge form - to print out and sign

(4) Photo - a recent (within 1 year) 1" by 1" color head shot photo on glossy photo paper players may not be wearing hats or sunglasses

(5) Registration fee - check made out to Triangle United for appropriate registration fee. Please write your child’s name and team name on the check 2011-2012 Club Fees Schedule

(6) US Club Soccer Medical Release - does NOT need to be notarized. Must be signed by player (once) and parent (twice) US Club Soccer Medical Release Form
